We are In-Network and accept most (AZ) United Healthcare/Optum Plans (Regular & EAP), UMR, AZ AHCCCS United Healthcare Community Plan, BHOEAP, Aetna including: Banner Aetna, GEHA, Aetna Meritain, EAP, and more . We have been approved and will be adding Cigna soon. We continue to add other Commercial insurance and Medicaid plans. Keep an eye on this webpage for updates. If you are unsure, please check with your health plan to see if we are in-network.
We are also In-Network: with Apostrophe through Sondermind at (844) 257-0989 or www.sondermind.com. If you have this insurance, please contact them directly to schedule an appointment and request: Deborah M. Johansen for your Provider Match.
If we are NOT in your network, please ask your carrier if they provide Out-of-Network Behavioral Health benefits. If so, we can see you for counseling. We will charge you for the session at the time of service and provide a Superbill for you to submit to your Insurance Company for Reimbursement.
**Unfortunately, we do NOT accept ANY combination of Medicare insurance plans.**
Insurance Questions
Many clients find that services are covered in part by their health insurance. Please be aware, if you have a deductible or copay, you will be responsible for paying those amounts at the time of service. Do check your coverage carefully by asking the following questions:
- Do I have Mental Health benefits?
- What is my deductible and has it been met?
- What is my C0-pay/Co-Insurance cost per session?
- Do I have EAP coverage and how many sessions?
- Do I need a prior authorization?
- Is there a limit to the number of regular sessions I am allowed?
Payment
We accept all major Credit/Debit, FSA, and HSA cards to cover services rendered including: Co-pays; Co-insurance, Self-Pay, and Late Cancellation/No Show Fees. You may submit payment directly through your client portal, or by calling Foundation for Healing, LLC at (480) 608-5250. (Contact the office with any hardship concerns.)
**Please note that all co-pays will be charged on the day of the first appointment, after verification of insurance, and then prior to the start of successive sessions. A $100 deposit will also be charged on your regular Credit/Debit card to reserve your appointment and/or to cover missed appointments/late cancellation fees. If not utilized, you may choose to apply the $100 to your last session, or have it returned to you, unless otherwise discussed with us.**
Cancellation Policy
We understand if you are unable to attend a session. Life Happens! Whenever possible, please cancel at least 24 hours in advance. If you reschedule at the time of the cancellation and more than 24 hours prior to the original appointment, the $100 deposit will be applied to your new date and time. If cancelled less than 24 hours, the $100 deposit will be forfeited as a late cancellation fee.
