A confirmation email will be sent to you within 24 hours. Once your appointment is confirmed, please click on the Client Forms link, at the top of this page, to return to the Simple Practice Client Portal and complete your intake paperwork. Please have all the documents filled out and uploaded prior to your session. If you are having technology issues, or do not have the ability to scan and upload the documents, please let us know in advance so that we can discuss an alternative way of getting them to us.
